Workshop Participation Agreement

WORKSHOP WAIVER AND RELEASE OF LIABILITY

Artfully Dunn Creative Studio

By registering for and participating in a workshop with Artfully Dunn Creative Studio, LLC, you acknowledge and agree to the following:

VOLUNTARY PARTICIPATION

You are voluntarily participating in an art workshop that may involve the use of specialized equipment, tools, inks, paints, dyes, adhesives, recycled materials, and other creative supplies.

ASSUMPTION OF RISK

You understand that participation carries inherent risks, including but not limited to:

•     Minor injuries from tools (such as scissors, heat gun, glass snippers, carving tools, or utility knives)

•     Clothing damage, ink or dye staining, epoxy resin, heat

•     Potential allergic reactions or skin irritation from art materials

•     Other unforeseen hazards related to the particular art that is being made

Protective equipment such as gloves will be available upon request, and participants are encouraged to communicate any allergies or sensitivities to the instructor before the workshop begins.

Participants are encouraged to research the particular art process they are participating in prior to attending the workshop

RELEASE OF LIABILITY

You agree to release, indemnify, and hold harmless Lisa M. Dunn, Artfully Dunn Creative Studio, LLC, its employees, instructors, affiliates, volunteers, and event hosts from any and all claims, demands, damages, or causes of action arising out of your participation in the workshop or travel to and from the event and that these terms serve as a release for me, volunteers, property owners and members of my family.

PERSONAL RESPONSIBILITY

You agree to follow all safety instructions provided by the instructor. You acknowledge you are responsible for your own well-being and safety during the workshop.

PHOTOGRAPHY AND VIDEO CONSENT

By participating in Artfully Dunn Creative Studio, LLC workshops, you grant permission for photographs and/or video recordings to be taken of you and your artwork during the event. These images may be used by Artfully Dunn Creative Studio, LLC for promotional purposes, publications, educational materials, social media, and website content, without compensation to you.

You waive the right to inspect or approve the final use of any such media.

Photography consent is a requirement for participation, as documentation is an integral part of Artfully Dunn Creative Studio, LLC’s mission to celebrate and promote community engagement in the arts.

If you have specific concerns or require accommodations, you must contact Artfully Dunn Creative Studio, LLC in writing prior to the event at artfullydunn5@gmail.com.

ACKNOWLEDGMENT

By registering for a workshop, you certify that you have read, understood, and agreed to this Waiver and Release of Liability. You acknowledge that this document affects your legal rights and that you sign it of your own free will.

PARENT/GUARDIAN’S AUTHORIZATION & WAIVER FORM

ABILITY TO ENGAGE IN ART ACTIVITIES AND ASSUMPTION OF THE RISK

Artfully Dunn Creative Studio, LLC takes all possible precautions to reduce risk and provide safe, healthy and enjoyable creative experiences. I warrant the Minor Child can follow directions for all activities in the studio/workshop. I acknowledge that risks from participating in workshop activities exist and that I have allowed Minor Child to attend art workshop knowing these risks and their possible consequences including personal injury.

PHOTOGRAPHY AND VIDEO CONSENT

I give permission to Artfully Dunn Creative Studio, LLC to use photos taken of the minor child I’m designated as Guardian (“Minor Child”) at the studio as well as photos of Minor Child’s artwork to be used for local news, marketing, social media - including Facebook, Instagram, and Twitter.

WAIVER AND RELEASE OF LIABILITY

As a parent or guardian of Minor Child, I agree that I will not hold Artfully Studio Creative Studio, LLC liable for any personal injury, property damage or loss of insurance. I agree to release and hold harmless Artfully Dunn Creative Studio, LLC and owner, Lisa M. Dunn, its employees, instructors, affiliates, volunteers, and event hosts from all liability incurred as a result of Minor Child’s participation in studio workshop and that these terms serve as a release for me, volunteers, property owners and members of my family.
